The parish church of Mulazzano lies on the top of a hill overlooking the surrounding village. The first documents, concerning the sale of land, which attest the presence of the parish church, originally dedicated to San Martino, are dated 1174.

The present building, dating back to 1630, consists of a single nave, barrel vaulted with nails, with a recessed presbytery that ends in the polygonal apse covered with sails, and four side chapels, carved in the thickness of the walls.

Externally it has a plastered facade, divided by four exposed brick pilasters with high base, and concluded by a triangular tympanum with central oculus. In the centre, in axis with the portal, is the rectangular window, opened in the 1950s, which illuminates the chancel on the counter-façade.

On the right side of the facade, stands the bell tower with clock on which is walled a sandstone tile of the eleventh century depicting a cross with the arms surmounted by two small disks in relief that were probably intended to express decorative motifs typical of the period while below, two heads decorated with moustaches and long pointed beards.

The nave ends with a polygonal apse and four side chapels, two on each side.

The church is dedicated to the Transfiguration of Our Lord.
